If I can blow my head gaskets stock at 18k miles, anyone can. If it was caused by warped heads, then anyone can get warped heads as mine were straight when the gaskets were replaced.
Let me answer your questions:
1-Run lots of power reliably
2-Put stock TTY bots back on.
3-Unlikely unless the dealer has no idea what they are doing or you decide to run boost pressures higher than 50psi.
