sounds like after he pulled the injector all the oil and fuel had drained out of the rails and into the cylinder. when he barred the engine over he some how had is face too close to the injector opening and the force of the piston coming up the cylinder shot the fuel and oil cocktail out at a high velocity possibly pierceing through his eye and fataly killing him.
at least thats what i got from the article.
