Could have been a valve spring and keeper, could have been an unbolted injector pushed out by cylinder pressure.....guess we'll have to wait for the full report.
I wish they would issue bulletins like this to automotive service shops on a regular basis to remind techs of the dangers of working around equipment.
I won't stand near a car while someone is listening to wheel bearing noise on the rack because I am afraid rocks might come out of the tires, I am paranod of any moving belts because while they don't often break, all it takes is once, or something fallen into it and getting slung back out at ya.
When I went back to Ford I had to get used to being around engine-driven fans again.
