I odered mine from an individual
($85 plus shipping - probably a little high for what you get, but the total cost of the mod was $158.19):
email NCHornet (walknmiracle@triad.rr.com) on:
Ford Powerstroke Diesel Forum
He makes the filter out of PVC and with his own filter media.
This is a good thread on the mod (with pictures):

Also - I have my own pictures and install instructions if you go this route.
You can also get a Racor Filter:

This is what NCHornet says about his filter (and I have found it to be true so far):
Since the filter media inside the filter is new, it may take 10k miles before enough oil is accumulated to drain from the pet **** on the bottom of the filter. After that, you should drain it every oil change. The amount of oil will vary from truck-to-truck, but on average, you should see 1-3 tablespoons in 5k miles. If you ever have more than this, then check to make sure you still have the "high point" in the hose around the degas bottle or master cylinder. If you lose this high point, more oil will leave the crank case and the filter will fill up more quickly.
The filter media never needs to be changed.
