fuel pressure, more specifically the lack of it. from what i've learned 40 psi is the absolute min pressure before damage occurs. (low pressure has several standard causes,#1fuel filter restriction due to dirt or water, #2 fuel pump getting weak or failing, caused by working too hard to pull fuel through a plugged filter). part of your problem is ford itself, the initial damage to the injectors probably happened at the same time but ford only changed the 2 very worst ones, hoping the rest would stumble along just above the margin for replacement. you probably want to change your filters more often and do like many other and install a pressure gauge that reads the pressure where the fuel enters the head, then you can see whats happening and avoid any more problems. the only other thing that comes to mind is if the copper seal at the tip of the injector is leaking and letting combustion gas into the fuel rail in the head, any void in the fuel will cause low pressure type damage.
