oil actually pouring out exhaust

my friends 5.4 started smoking white at first, now its running horrible and there is black sludgy oil coming out of the exhaust...dripping

is this a sign of a head gasket or broken piston..etc? the motor sounds ok, no nasty slapping or knocking, just running rough

where would the oil be coming from?

1985 F-350 Diesel 6.9 4x4 Reg Cab SRW c-6 automatic connected to a borg-werner 1345 TC, 4:10 posi d-50 front and sterling 10.25 rear, TTB, A/C, new springs, shocks, starter, 450amp alternator new floors rockers wheel wells, doors, hood, fenders & black paint

accident in feb. 2006 left her totaled, but will be re-bent to perfection in time.

dads: 1992 non-turbo 7.3 e4od 4x4 extended cab 3.73rears 265k miles, runs like crap, looks like an angry cloud.
