98% of the melted pistons are caused by over full crankcases. At a certain point oil gets sucked into the fresh air inlet tube via the crankcase breather tube, fills up the charge air cooler with oil/fuel, and will work it's way into the intake manifold. Usually takes out the left bank, cylinders 2 and 8 first. It's usually caused by an external fuel leak at the injector fittings or the high pressure fuel pump itself. I've had others where it's just a total lack of maintance and the truck runs 16 hours a day (1 hour equals 25 hours). The truck would have 12,000 miles on it but 880 hours, so thats 22,000 miles without an oil change. The crankcase would get overfull for the excessive idling and have the same effect.
