15mpg in-town is normal for me, closer to 16 usually. I have the same truck. 20 or so is more common on the highway depending on wind direction/vehicle speed.

This is you exhaust back pressure valve. It helps your engine warm up when it's cold. Truck will feel sluggish when it's on. Perfectly normal. If it does not go away once the engine is warm, then you could try unplugging it.
