A PTO (Power Take Off) is a gearbox that bolts to the side of the transmission and usually meshes with the reverse idler gear. I would assume that your transfer case also has a PTO opening, and the oil level would need to be higher in order to get lube up to the PTO hole and into the PTO gearbox.

For a PTO lesson, my tow truck has a PTO mounted on the side of the trans which runs a hydraulic pump to run the various wrecker cylinders. Hydraulics are the usual purpose of PTO's, although they can be used for water pumps etc. You can basically use them to drive anything that needs to turn and can be mounted under the truck.
