I had to get 2 extra keys made for my ex 2004. I went to the local locksmith and it was around 50ish...be sure to take all of your keys when you get any made so they can all be programmed the same.
I did this since I have a aftermarket remote start alarm..to start the truck a key has to be hidden somewhere in the truck..mine is tucked way back under the dash somewhere..its not visible or anywhere near reaching since all the dash was apart to install the alarm FYI.

no, you do not have to go to the dealer.......they now have a way to clone your key...check with a locksmith.....if they cant clone the key, then they should be able to program another....i charge $50-$75 for everything.....the prgrammer cost around $5000.00.....but i am also a locksmith so it was worth the investment.....with the programmer, they can erase all other keys so that they dont work anymore.......

It is in the manual, just follow it to the letter.
For those with after market remote starts I used
Ford part number 1L3Z-19G365-AB KIT-PSV # 555608 with my non ford/codealarm unit
The second number may be the code alarm number
It is a little box and coil that goes around the lock cyl.
You "teach" it just like a key is is less hassle than the key in the box and the required relay if it is done right.
Cost me $30 from Dealer
If you do have the "key in the Box" be sure to cut off the end of the key so it cannot be removed and used to drive your truck away!

You need to have 2 programed keys to do this.
Insert the first programmed ignition key into the ignition lock cylinder and turn the ignition switch from OFF to RUN (maintain the ignition switch in RUN for one second).
Turn the ignition switch to OFF and remove the first key from the ignition lock cylinder.
Within five seconds of turning the ignition switch to OFF, insert the second programmed ignition key into the ignition lock cylinder and turn the ignition switch from OFF to RUN (maintain the ignition switch in RUN for one second).
Turn the ignition switch to OFF and remove the second key from the ignition lock cylinder.
Within 10 seconds of turning the ignition lock cylinder to OFF, insert the unprogrammed ignition key (new key) into the ignition lock cylinder and turn the ignition switch from OFF to RUN (maintain the ignition switch in RUN for 1 second).
If it is desired to program additional acceptable PATS key(s), repeat the applicable key programming procedure from the beginning.

I bought my used X from Carmax and it had only one PATS key. Carmax agreed to pay for me to get another and my dealer charged $79 with tax ($30 for the key, rest for labor.) The key they sold me was a Strattec and then I bought two more of these on eBay from locksmithacedog for $18 delivered, for both. To cut them, go to a store with an Access key system and use cartridge A as the non-PATS key is #24 in their system. If you are nice, or just lucky, they guys will not even charge you to cut the key because you are not buying one.
For the FOB, I bought two of these as well on eBay from mykeyless.. a new one for 14.95 and a used one for 8.99 and shipping was about $7 for both. You can program all of them yourself easily.
