Why is the 3.73 LS apparently not available (according to Ford Fleet 1 web site) in a 5 speed 4x4 F250 V10 whereas it is available in the F350? Is the only LS gear for the F250 4x4 5 speed the 4.3?

Yep, you got it exactly right. I don't think there are any 'techincal' reasons for this. My guess is they had to make choices of how many combinations to support, in order to manage costs. (auto/manual, 3.73/4.10/4.30, LS or not LS etc)
