2003 Shaking, bouncing, missing....

My 03 just started shaking. I was driving at about 15 mph when the truck all of a sudden started shaking like a cylinder went down. It feels as if the problem is an injector but the computer will not flash any codes. There is a small amount of white smoke coming from the exhaust. I was able to drive the truck home (about 10 miles) but when I eventually stopped, the shaking of the truck felt like it wanted to die. When I give the truck some throttle, the truck revs up great other than the shaking. Does anyone have any ideas? I know that it is not the EGR valve. Thanks in advance.......

Need to add............. the truck made a big clank sound and then the problem occurred. The clank might be coincidence, I noticed the shaking about 1-2 minutes after the clank.

OK, I just started the truck after it has sat for a full day. The truck started up like normal. No shaking, no missing. I let the truck warm up for a few minutes then I hit the throttle and kept the RPM's at around 2000 for 15 seconds. When I let off the throttle, the shaking started. I let the truck run for maybe 2 minutes and then shut it off due to the shaking not stopping.
