Broken rocker arm and push rod.

Its been a while since I have had to post but this one has me really wondering. I bought a 04' F250 with 240,000 miles from an auction and am finding lots of fun issues. I had rough idle and shaking at low RPM 45-60 MPH. Pulled Unbalanced contribution cylinder 3 code...did some searching and came to conclusion it was an injector. So I tore into it and found exhaust rocker arm broke and dug two halves of the intake push rod out.

So my question is where do I go from here? What may have caused the issue? I know nothing about the history of the truck and to the best of my knowledge it is completely stock. I did also pull an EGR code, the oil dummy needle drops to zero after reaching operating temp and when pulling the hose for MASS air sensor the fitting was plugged with a dry sludge.

Also are there supposed to be 2 tubes that run through the head for the high pressure oil? I had a friend who has worked on these before say there is supposed to be 2. I have one in the back but nothing in the front and no clue where it would be if it isn't in the hole.

Pull your oil filter out, cut out a piece of it, use a vice to squeeze oil out and check for excessive iron in pleats of filter. If you have metal in filter, you have internal failure. Have seen several with rocker arm failures, which could lead to broken rockers/push rods.

The 03-04 year has one standpipe per rail. 05 and up has 2Broken rocker arm and push rod.-03oilail.jpg

In 04.25, they went to the waved oil rail...it had 1 stand pipe and 1 oil rail plug.

broken rockers,and bent pushrods can happen as a byproduct of reassembly after having the heads off. replace the damaged parts, roll the engine over by hand 2-4 full turns to make sure its free before cranking it up

Just wanted to follow up saying I replaced the broken parts and the engine is up and running. Having some transmission codes come up but I'll start a new thread for that if I don't find the answers after some research of my own. Thanks for the advise.
