Feels like my 6.0 is missing

So here is what I have, 2004 F 250 6.0 auto with 65000 miles. This week on vacation towing a 2 bike motorcycle trailer while going up grades at about 65 the truck starts to miss and feels like it is spitting, if I put my foot on the gas the miss goes away and it accelerates fine. I almost seem to hear a coughing sound while it is missing and I can feel a hesitation. Only shows the issue up hills. Always goes away once I put my foot into the gas. Any idea what's going on? I change my oil and filters every 6K and fuel filters at 12K.

My 450 has had a random misfire for 80K miles. Never sets a code and has not gotten any worse. I have to be loaded to feel it and I have gone to dealer with it loaded and drove with tech doing graphic display of crankshaft speed deviation with IDS while I drive in places where it usually will act up. It didn't act up while he was monitoring.... Fuel pressure is 60 psi when it occurs and FICM has been replaced (not long ago) and the problem is the same.
A friend who works at a Ford dealer says they started seeing the misfires like this and usually progresses and injectors fix it.
Mine has been long term and my injectors have not developed any symptoms (except for one that failed 40K miles ago) so I keep going until something gives.

DO you have any other symptoms like morning rough running?

No other problems that I can tell, I do get some white smoke when first taking off in the morning when it's about 10 degrees or coder out. The smoke goes away about 2 minutes down the road. The truk starts up no problem and has not given me any problems.

When that happened to me it turned out I had an injector going bad. Have the dealer load test all your injectors and I'll bet they find one of them isn't within spec.

Well the problem seemed to go away as I havnt been towing since Jan, however this past weekend I took the travel trailer and a dirt bike to Terlingua Tx for some R&R.
On the way home the problem returned, felt like the truck was missing and spitting pulling the hills. Stuff my foot in the gas and the miss went away.
Took it to a local diesel shop I found in Santa Fe.
They just called and told me the EGR valve was clogged up. They have a cleaning kit and process for about $ 200.00 that will fix it up, so they claim. I told them to get er done so I will let you know the results.

Well my truck is fixed and it had a slew of issues. I just had to pay $ 1,800.00 in repairs. It had a bad OBD2 port so the computer would stop giving codes. That drove the mechanic crazy as he couldn't get an accurate read on any codes, once that was sorted out they found out it had a bad alternator and the EGR and MAP sensor needed to be cleaned. It took the shop about 20 hours to figure everything out and they charged me for 8 hours so I think they were dam fair in pricing the work out.
What do you think?

This sounds identical to my problem I was having with 2003 6.0. Like mentioned above it was injectors on mine as well. We had them tested and new injectors fixed the problem.

Mine tended to do when it cold more than anything and when you added throttle it cleared up.

So it looks like the issue continues...
The OBD2 plug has been repaired and the Edge gauges stay on but the truck started doing the same buck/miss problem yesterday whae I was going up a hill at 65 without the trailer, I gave it a little pedal and it started the buck/spit/miss until I put my foot in it and then the problem went away.
Did it a few times during the day. I am getting the following codes
P0404 EGR Valve
P0488 EGR Throttle Position Control
I will be calling the repair shop tomorrow but what do you all think the solution is?
Replacing the EGR instead of the cleaning they did?
Thanks

Captrick, I had the same symptoms and couldn't figure it out. However, about 800 miles into looking for it it developed a "surge" where I would be sitting with my foot on the brake and truck in gear and it would jump forward. Like a gremlin had stood on the throttle for a split second. took it in and had the ICP senspor replaced to fix the new problem, replaced the oil cooler and did the EGR delete and havn't had that problem since. I chalked it up to the ICP sensor being the culprit on the intial coughing problem, but it could have been coincidence and maby a loose conection got plugged back in better or something.

Delete the egr. Also have the ficm checked as well. Also oil cooler could be clogged or going bad.

The shop put in a spare EGR valve they had in the shop.
Symptoms are gone, so they ordered up a new EGR. With the spare EGR valve my mileage went from 15-16 to 17-18+.
