Sounds like it could be injector related. Do you have a way to pull codes or do troubleshooting (like with an auto enginuity)?

Not usre if your has it but could it be the exhaust backpressure valve defective?

Not sure if that could cause it.....maybe not but that was the first thing it came to me since it is suppose to close up to help warm the engine faster, is it very cold where you are? It probably is now...

The SCT will not read all the codes. I am beginning to believe that as soon as the truck is past warranty, we should all buy an AutoEnginuity (probably help even before if you have to pay a few deductibles)!

TOEN -
The first place I would start would be to pull the EGR valve, clean or replace if necessary. Check the MAP sensor hose and the EBP tube for pluggage. Clean if necessary.

After that, find a way to run injector tests (cylinder contribution test).

MAP:
The MAP port is on the intake manifold. The hose comes off of that port.

You did pull the EGR valve and clean it? If so, what did it look like inside?

Also - do you have any aftermarket modifications (air intake, tuner, etc)?
