i will also consider the ficm if the hot shots does not take care of the issue. i hope we all will continue to post our findings, this is getting pretty frustrating and costly

Here is some FICM info for ya. It may help give you an idea of what you are dealing with diagnosing it.

I replaced my FICM because the voltage was way below the 48v needed! You should check Your volts to the FICM before just thinking it might be the problem. I will tell You that my FICM went bad, after it was repaired by SWAMPSDIESEL I still experienced the mild stutter at or around 1300-1600 RPMS. I checked with some heavy hitters on this site and others and can tell You that I am having an injector issue. This is specifically My truck and may not apply to yours. I have put HSS in my oil and can tell that it is in fact making a difference after the 900 miles that I have run it( Stutter seems to be lessening and truck is running good) I also run the two stroke oil in my fuel. Bottom line: Don't replace FICM for the Hell of it! Check the Volts FIRST!!! Merry Christmas.

Merry Christmas and thanks to all. When I drove it yesterday; the first stop light (14mi. from house) it idled a little rough. It did not buck on the highway. Next town (16mi.) rough idle at stop light. Drove through town to store smooth idle. 10 more mi. to inlaws no problems. When we left I tried to make it buck. It would but now only above 2000rpm. As soon as I get a scanner I guess I'll be busy checking every thing I can. The only differance between yesterday and the day before ; 20-30 deg lower air temp and I had unplugged/plugged in those sensors. I think its jumping around as far as what rpm it will act up.

Merry Christmas to all. I've been dealing with the same condition except with the addition of an infrequent engine stall out since July. It will always start right back up with no check engine light (cel) and with out a code for diagnostics. I've had turbo rebuilt for "coking" @ 39k back in July according to the tech, EBP sensor with new connector soon after, new alternator due to infrequent voltage drops in Sept. As of a week ago conditions resurfaced finally with a diagnostic code showing a problem with ICP sensor. Sensor was changed along with connector. Picked truck up a couple of days ago and feels very responsive. We"ll see for how long.

OK I ran some tests. FICM Volts at 20deg air temp. was 48.5-49v. I checked it with the AutoEng. I also checked it driving down the road. It went as low as 44v while driving. ICP was compared to ICP desired and they matched the entire drive. I also retrieved a P0275 #5cylinder contribution/balance test. I believe that was the code. I failed to write it down. I'll check again. It came up twice after clearing all codes. During the test drive I monitored the different missfire modes I could with nothing showing.

I was researching this problem on another site and found a guy that had the exact same symtoms I am. After three #5 injectors he took it to another shop. After some head scratching they found the exhaust cam lobe for #5 almost gone. New cam and lifters, he's good as new. Is there any way to check this with out tearing down the engine?

I'm getting P0275 and P0200 ocationally. It has also developed the cold start problem(it runs like crap until warmed up) other people have. I some how missed the IAT from previous posts(checked it last night and it was gummed up with crap) cleaned it as well as changed fuel filters. Wife said it ran like crap today. The cold start issue seems to be getting worse sinse it's only been in the 50's in the morning and 70's during the day. What is odd is with the scanner hooked up while driving, the scanner doesn't show anything when the truck acts up. Does anyone have diagnostic steps for these codes?
I just noticed the link in the previous post for FICM diagnostics(thanks). I'll check that as soon as I can. I guess I better reread this thread for anything else I missed. We are in the middle of livestock show season for my kids and I'm studying for a promotion exam. What timming. Thanks to all.

Have you had the fuel pressure checked. Could be a fuel pump issue...

IAT sensor is dirty from breather soot. That sit on the manifold next to the upper fuel filter.
Also change the ICP sensor that sit on the pax side. Should fix the bucking blues.
