Mine started doing that a couple years back, recently replaced the HPOP with the Adrenaline, and wha la, no more smoke puffs at startup.

What color is the smoke? White/grayish? If so it's probably a bit of raw fuel, it could be from one cylinder not burning the fuel completely for any reason, from a bad injector, (Sounds serious, but it isn't) to a low compression cylinder. (After warm up, it seals a bit better)
I have never heard of an HPOP issue causing smoke at start up... How would that cause it? BEFORE you suggest someone go out and spend 500+ dollars an explanation would be nice!
I imagine if yours had a small puff of smoke at start, you COULD have fixed it by replacing one injector I bet, slightly better HPOP than stock may have helped cover it up, but I would guess you still have a less than healthy injector in there.

The stock HPOP RARELY goes bad, they are very dependable for daily driving, NOW if your wanting to increase performance, ect.... there are several options out there.

The HPOP is a High Pressure Oil Pump. It supplies your injectors with anywhere from 500 to 2800 lbs (STOCK HPOP) of oil to hydraulically work the injectors. (Performance HPOP systems can go over 3500 lbs)
Brief explanation,,,,,,

The fuel is supplied from the tank at about 60lbs of pressure to the fuel water separator/fuel filter. And sent from there to fuel rails INSIDE the head. Each injector has a port in the fuel rail where fuel enters the injector.
The HPOP is located in the valley under the fuel bowl, it has a reservoir (right in front of the fuel bowl) where less than a quart of oil is circulated from the pan with the "Regular oil pump) LPOP to it. There are lines going from the HPOP to the heads also, where the oil passes through like the fuel also into the injectors. This oil can be anywhere from 500 lbs to 2800 lbs on a "Stock truck" depending on demand from the electronics. The PCM and IDM, (Powertrain control module, where you would install a "Chip" if you got one, And Injector driver module) are what electronically fire the injectors. The oil pushes down on a piston inside the injector to compress the fuel in the bottom. When the IDM tells the injector to fire, the solenoid opens the injector and it comes out at THOUSANDS of pounds of pressure, (I believe, 19,000 lbs, but that is not important to understand the general operation) The HPOP numbers are fairly important though, if you get one, and work on it yourself, those numbers will help if you have trouble down the road trying to figure out the problem.

For 5,000 i'd say its worth it if its not trashed inside or rotted out. That seems really cheap for a dually. My truck does the same thing when I start it cold. Pull of the intake boot and check the turbo blades to make sure they're not worn from dirt ingestion though. Its very common on our trucks, especially if the stock air system is in place.
