You'll also need the following...did mine last August (2008)
1. Brackets for new bumper
2. Valance Panel
3. Fog Lights (Yes...they are different than the 99-04 style)
4. Headlight Wiring harness (they used a different bulb style in 05...there's a harness avaliable at dfuser.com...it will make the new headlights plug and play)
Not trying to hijack this thread but, if i were to just get the grille and headlights from an 05 Harley, will I be able to just bolt it in? If not, what problems will I face?

Headlights will work but you will have to cut your existing header panel to make them fit. As well, you will have to replace the bulb connector harness to the newer H13 style. The grill will not fit as it is bigger than your 04 and will interfere with your bumper. However, some have cut the bottom of the grill to make it fit.

Was the conv a long/hard process or what it moderately easy?
You do not need the bumper brackets (referring to the side ones). The four bolts that hold the bumper to each frame rail will be sufficient. The upper valence (bumper cover usually painted to match) should come with the bumper.
The conversion is a simple bolt on procedure. You will have no problems getting it done. See this thread here
