I never have pulled with the truck before i did the injectors but it used to be whenever i had the 4wheeler in the back i could feel a slight shutter with the extra weight in the truck.. the when towing a 10k lb gooseneck it was even more noticible and would shake the truck profusely.
after making a very aggressive acceleration run heading to the fire hall for a call its noticible even with no extra weight in the truck in 2nd/3rd/4th and not very noticible in gears that will create boost very quickly.
it feels like a miss possible but at idle and a neutral rev theres no sign of it. only under load does it does this.
so far trying to remedy this ive done new valve cover gaskets/glow plugs/uvch's
it does it whether my adjustable 16k mod is on or off.
truck is 500miles from rolling 200k on the odo. when pulling harnesses off the injectors at idle all create a rougher idle. but i also noticed on 2 pass side cylinders i could not c any oil coming out the spout granted my lighting was crappy also when trying to look.
when taking off with 16k mod turned up all the way it will blow black very aggressively sometimes it looks more blue though.
when doing the uvch's i pulled all driver side solenoids off and checked for tightness of the poppet valve plate. i did not check the pass side though but plan to do it later today.
to make it clear whats been done to engine is roosewood DIY stage 1s, elec fuel inject, 16k adjustable and on/off switch mod, switched from auto to zf 5 s42 from 86 f-450 i have no gotten a manual computer or harness am using auto harness with MLPS set in neutral so it lets me start the truck.
the bucking/shutter was felt also the e40d
any ideas would be great hopefully have enough info here to make a decent educated guess.
also 4500 roughly on the oil with very minimal towing plan on changing oil today also and sending sample into blackstone labs
when injectors were done all o-rings were replaced and internal rings also
truck since ive owned it has never been ran out of diesel but has been cranked with no oil in res.

There was a TSB about the torque converter causing shutter. The prescribed fix was to change filter and fluid including draining the torque converter. There is a plug in the converter. You have to remove the inspection cover and turn the engine over with a bar and socket until the plug is facing downward and then remove the plug. My buddy who works at Ford uses a coffee stirrer(s) stuck in the hole to aid in draining them. You should also flush out the cooler lines going from the trans to the radiator.

Sounds to me like a dual mass flywheel starting to let go... I had exactly how you describe it, a shudder under load.... performed cylinder balance test, checked wiring harnesses, changed those and valve cover gaskets.... turns out it was the dual mass flywheel. Pull the inspection plate off bell housing and you shouldn't be able to move the clutch back and forth too much.... there may be debris in there aswell (ball bearings etc if it has come apart). Replacement with a single mass flywheel is your cheapest and best option. Let me know if this helped.
