I replaced mine under regular maint about 75K ago. Monday night it completely failed, and spit all the rollers out of it.
I drove another 100+ miles like this beating the races to a pulp, but thankfully did not damage the shaft.
What causes these things to fail?

UPDATED 8/1/08 Replace so far. 1 LUK flywheel+clutch, 2 thermostats, 2 set of brakes, 1 set of calipers, 5 CPS, 3 sets of tires, 2 Transfer pumps, 1 Injector modual, 1 Computer, 2 Alt, 2 sets of batteries, 1 Water pump, 6 Belts, 1 PS hose, 2 Sets ball joints, 2 set u-joints, 2 carrier bearing, 2 Speed sensors, 1 oil pres sender, 1 temp sender, 4 sets of e-break cables, 1 front fuel tank, 2 rear fuel tanks, 2 set of glow plugs, 6 Glow plug relays, Oil galley o-rings, Turbo pedistal o-rings, EBPV o-rings, 3 sets of Injector O-rings, 1 Vac-pump, 1 new carpet.Total $$$ in repairs v/s miles driven = 3.0 cents per mile. Add fuel to that it jumps to 14.8 cents per mile over the life of the truck.
Chinese or Mexican / Brazilian Carrier Bearings fail prematurely along with what Ammayernick said is 100 % true . When you installl the new one make sure its alligned and balanced and most of all Made in the USA or CANADA . Sometimes you have to shim it out with Washers.

Is the truck lifted...I would think that would cause a carrier bearing to fail fairly fast. I know when I lifted mine a had a vibration off the line coming from the carrier bearing.
I know some people with lifted truck had a custom 1 piece drive shaft made to fix the problem...others just put in a drop bracket
Of course this is all competely useless information if the truck isn't lifted...

Truck is not lifted. There is no vibration up to the point it puked it guts out. I replaced the first one because it had a little play in it. Nothing in the info says anything about shimming? No shims came off and non were included with new ones. Maybe your thinking the pinion? Preload? All it does is press on. There is no option for preload?
Even after it puked its guts out there was very little vibration.
I guess when it is time to do joints I will get the shaft ballanced?

I have never lubed it. I guess I could buy a needle to get grease in there, but I thought it was life time greased?
I don't think I ever gave it a drink, but who knows.

Parts normally come from NAPA or Sanells here in NH. I am sure it gets wet frequently, but I don't think it was ever submerged.
I have never has a schedule for that bearing. OEM one never had a zert. First replacement did not. Next time I am under it I will look at this one. They have needle grease tips. I have thought about buying one of those becuase of u-joint cups. I guess this is another reason.
