All 6.5 rounds are not 6.5, if you get what I mean.
The rears are not 5x7. They are some wierd size like 4x6. With a little cutting and bending you can fit a nice 6.5 in the front (which is larger then 6.5) and actual 5x7 in the rear. The rear is a little tricky to fit in there. It will look a little crappy until you put the plastic back on, but then it looks great.
Make sure you get 3 way speakers. They sound much better than the stock 2 ways.

hmmm... crutchfield says 5x7 for rears in ext cabs, but the 6x8s for reg & crew cabs. i have installed speakers in all the configs of obs cabs & all had 6x8 rears.
that being said 6x8 is a fairly terrible sounding speaker unless you put an amp to it. it wants to think its a big speaker, but can't handle low freqs like one. i would get an adapter & a higher quality round speaker for the rears if i was doing it again... or if i could ever blow these things.
a little 10" with nice 6.5" all the way around would sound pretty good =)
my 97 standard cab has jbl 2 way 6.5" rounds in the door and and jbl 2 way 5x7"s in the rear corner panels. i have an kicker 8" sub in a wedge enclosure with a kenwood bridgeable 200 watt amp. jvc el cameleon amfmcd player. plenty of power from the head unit. sounds great! if you have the stock stereo it will not sound good. it does not have enough power for aftermarket speakers! its all about how much power you feed to the speakers.

I was thinking a 6x9 was a 5x7. Anyhow if you work at it you can get the 6x9 to fit in the rear hole.

I put 6 1/2" in the front with a little massaging of the opening for them to fit.
I did buy 6x8 (factory size) for the rear, the 6x9 looked like it would need too much modification of the hole to get in for my taste. The 6x8 Pioneers have done very well for me the last 6 years.

you will get better sound out of a nice pair of 6.5" over the 6x8s. 6x9 is an exceptional speaker, but for the most part round speakers out do their oval cousins.
