What you need is an OBD-1 code reader, with a connection specifically for Ford. I got mine at the local parts store for $30 US. When you're using it remember that it will read only the transmission computer, nothing about the engine since the engine has no computer on an '89. Somebody else will have to tell you where the connection is for a van as I have no idea.
