I have 285-75R tires, 5-speed and 4.10 gears and get 57 mph at 2000rpm. I always start off in 2nd w/ no problem, unless I'm towing or starting up a steep hill. Then I use 1st. Don't know if that helps, but I'd like to change to 3.55 or 3.73 gears for a little better mileage.

On my truck I used to have an E4OD and a 3.55 rear axle running the stock 235 / 85's. It felt like I had way too much top end and not enough power starting off. I simultaneously switched to a PSD 6 speed ZF and 4.10 rear (same tires) and now feel that was too big of a jump. I think a 3.73 would be perfect for me... It doesn't hurt that I have so many low gears now with the ZF6. But, I am almost always in OD now if I'm going over 40mph...
To see how different gearing / tire size combos affect engine speeds try this RPM / MPH CALCULATOR

Same here except 235/85R16s (pretty dang close circumference wise I think )... depending on how much grade is in front of me at the time, I occassionally use 1st to get rolling.... other than that, usually 2nd to start off in.
Best as I can tell... 13-15mpg in town and around 16-17mpg on the highway (*ain't* complaining! My brothers 2500HD w/8.1L gasser gets 12-13/8 towing! )
