Sad, but it a reminder to be careful. Who knows what the "expelled" material was? The copper washer under the injector? The oil itself? I'm lead to believe he was not wearing his safety glasses, and got shot in the eye with the oil and or something else. Since it said he was looking at the lifters.
sounds like after he pulled the injector all the oil and fuel had drained out of the rails and into the cylinder. when he barred the engine over he some how had is face too close to the injector opening and the force of the piston coming up the cylinder shot the fuel and oil cocktail out at a high velocity possibly pierceing through his eye and fataly killing him.
at least thats what i got from the article.

Could have been a valve spring and keeper, could have been an unbolted injector pushed out by cylinder pressure.....guess we'll have to wait for the full report.
I wish they would issue bulletins like this to automotive service shops on a regular basis to remind techs of the dangers of working around equipment.
I won't stand near a car while someone is listening to wheel bearing noise on the rack because I am afraid rocks might come out of the tires, I am paranod of any moving belts because while they don't often break, all it takes is once, or something fallen into it and getting slung back out at ya.
When I went back to Ford I had to get used to being around engine-driven fans again.
While tragic, I do not understand the topic of your post?
We have alot of DIY guys on this website that work on their PSDs(similar HEUI injection system), Its just a reminder that something that seems simple enough to do, can take you life away.

My tire guy will still balance tires on the vehicle he tells a story about a tape weight flying off of a wheel spinning about 65 MPH. and hitting him in the gut. left a bruise that was simmilar to what hes seen from bean bag bullets use for riot control.
Rotating shafts and electric arcs are something i have always been Leary of.
