isuzu vs cummins 4bt

I am gather info on a swap for my 86 Ford econoline. Many people have suggested the cummins 4bt (the van will be a land yacht doing 90% freeway driving so am hoping to get good mpg). Several people have said that the cummins rattles considerably and might not be appropriate for a passenger vehicle and that the isuzu is a lot quieter.

What's the scoop? Is the isuzu quieter? If I find a cummins block and rebuild it from scratch (or close to it) can I get a smooth running engine?

I drive what basically is an Isuzu box truck at work, I wouldn't wish that truck on my worst enemy. It could be they're using it for it stinks, not really sure, I would have bought an Int'l that a different story. I'd go with the cummins.
