I have a '92 E-350. It started blowing smoke in overdrive during deceleration driving downhill, with a slight shudder. At first it only happened during deceleration, now also during accel. When accelerating at high speeds, mostly after downhill coasting, during acceleration it shudders and smokes and has little/no power for a short time, until it feels like something catches. Then the smoking stops and the shudder lessens and power feels more normal. Not sure if I am being clear.

Seems to run fine with O/D off, however it has continued smoking slightly more than usual.

Any guidance would be appreciated

What color is the smoke? Is it possible to take a wiff of it somehow? Leaky coolant has a stink all its own. Have you checked the coolant level? I am just learning about diesels but I am wondering what a compression check might reveal. You may need a special diesel compression gauge for that. Wonder if you have a miss-firing cylinder... wonder if a remote temp gun would reveal that.
