Just wondering if you found the solution to this issue. My 2007 6.0 (all stock) is doing the same thing.
I took my truck to a new mechanic about a month ago. They had it for about 2 weeks and had a hard time finding anything wrong with it, even though it was obviously overfueling. Eventually, they pulled an injector and had it tested. Ended up having 7 injectors not flowing right, so I got a full new set. While they were in there, they replaced a couple of pigtails that weren't in good shape. It seems to be a lot better, but there is still something wrong. I've only had it out a few times since I got it back, and it mostly runs well, but the last time it was suddenly rolling coal again. I'm now getting a p0406 code, which I never got before (I've never had any engine codes before this). I'm now thinking it may all be due to a bad connection between the ECU and EGR valve, but I'm still not sure.

Finally found the solution to my black smoke issue. It was a lagging EGR valve. Had a EGR eliminator installed (not a delete). My truck doesn’t throw a code without EGR. Found all this out my unplugging my EGR at the top of the valve (no code, no more smoke)

What the heck is the difference between an EGR delete and an EGR eliminator!!??

Great info in this thread! I'm trying to hunt down a similar issue. So did you test by unplugging the EGR connector? I have a feeling my valve is also laggy because it was tested a while back and while it actuated it was following right at the test line.
