Side to side play - rebuild it. End to end - probably ok to let it go until your quarter million mile tune up. I would suggest evaluating your filtration system. I switched early to the Ford Severe Duty AIS and my wheel looked nearly new when I rebuilt mine. (Of course, it retired when I put on the RiffRaff billet wheel.)

So would I just replace the compressor wheel or do I need to rebuild the entire turbo? Sorry I’m new to turbo diesel engines. Looks like a Severe Duty AIS is in the future for me because I live in the desert.

If one has excess "side to side" play that is a radial bearing issue, changing the impeller wheel alone won't change that bearing element condition.

That bearing scenario makes the wheel itself a might as well and the rotor bearings a must.

Imbalance? That's defined as an unequal distribution of mass rotating about a principal axis, it's not a cosmetically definable certain condition. It's mass distribution not looks.

End-to-end play is a worse condition than a little radial play. There should be NO end play discernable and barely a click of radial play when you grab the compressor wheel nut and try to move it. End play means the thrust bearing is worn and may cause the compressor wheel to hit the housing, as well as take out the oil seal.

I would do a rebuild that turbo, with a new compressor wheel, bearing assembly and get the bearing/wheel assembly balanced.
