We will be doing a lot of mountainous hauling in our E350. Does anyone recommend an exhaust brake? What will work on an E-Van? Where does it install? If, for example, I get an aftermarket exhaust will it affect where/how I can fit an exhaust brake?

Banks might have one that installs close to the turbo, but it might not fit under a van's engine cover. There are several variants available that plumb into the exhaust, where the cat is or just ahead of it. They are usually sized for 4", so you might as well do your exhaust at the same time. If you don't want to, you can just put two adapters on it.
