transmission shudder, is that like when you are driving a standard trans with a grabby clutch in between shifts or just a bad driver?

If so, can flushing out the old trans fluid help to eliminate it?

I have a early 99 with 168k on the original trans. It shifts great when unloaded, but it will shudder (I guess) once in a while when pulling.

normal temp unloaded: 70* above ambient
normal temp towing 7k - 18k lbs: 80-90* above ambient
never seen temps above 195*

yeah, always durring a gear change.

mostly durring 2->3, once in a while durring 1->2.

3->4 still feels normal as well as all downshifts.

A good vb like the ftvb or Bts might help a bit, but the thing is prolly gonna need some internal work before its all over. Sounds like the clutch packs slipping on the gear changes.
